Verification of Independent Expenditures: Instructions, Reports & Form 462

Beginning January 1, 2013, campaign committees that make independent expenditures of $1,000 or more must verify that the independent expenditures made by the committee are--in fact--not coordinated with the relevant candidate or measure and that the committee is reporting all contributions and reimbursements.

Form 462 is a one-page form that identifies the committee making the independent expenditures and the relevant candidate or measure supported or opposed. The form is signed by a principal officer, candidate or state measure proponent.

Submit completed Form 462 via e-mail to: form462@fppc.ca.gov

The acceptable file format for submission is pdf, all others are invalid file format (i.e. jpg, .jpeg).


State Elections 2014

Select the specific office or measure to view Form 462 information in an Excel format. The spreadsheet is updated on Wednesday and includes Form 462s received the previous week.

Local Elections 2014

The FPPC will not post forms filed in connection with a local election.
To receive a copy of a Form 462 for a local election: email form462@fppc.ca.gov.

You must identify:

  • the city, county or special district (e.g. San Diego City or San Diego County);
  • the name of the candidate or measure (e.g. Councilmember Lee or Measure A) and
  • the month and year of the election.
