Form 700 - Statement of Economic Interests
FPPC staff is available to answer any Form 700 reporting questions you may have. However, if you are a conflict-of-interest code filer, you must first have your disclosure category before we can provide reporting assistance. Often times your disclosure category limits your reporting requirements. To obtain a copy of your disclosure category, see your supervisor or staff in either your Legal or Personnel department.

Form 700 Disclosure - Consultants & New Positions
In September 2011, the FPPC adopted two forms for agencies to use to document filing requirements for individuals serving as consultants or in newly created positions.
- Form 804 for newly created positions.
- Form 805 for consultants provide a user-friendly format for agencies to assign disclosure requirements.
Disclosure obligations should be tailored to the duties of the position or contract and apply to those individuals that make or participate in governmental decisions.
Statement of Economic Interests - Form 700 Late Filing Penalties
The guidelines provided in the following document apply to individuals whose Form 700 is forwarded to the FPPC. These statements include, but are not limited to, certain state and local elected officials, judges, high level state employees, and certain employees for the assembly and senate. Failure to file a statement will result in a referral to the FPPC Enforcement Division. A penalty of up to $5,000 may be imposed. File on time!
